Policies of the Office

  1. Controlled substances will only be prescribed in up to 30-day quantities unless otherwise indicated. Your doctor may decide to write smaller quantities if clinically indicated.

  2. Please be advised: if you have contact with your doctor between regularly-scheduled appointments and/or outside of regular business hours, you may be charged for a Telehealth session.

  3. Unfortunately, we are NOT able to offer:

    • Qualifying diagnosis evaluations for medical marijuana.

    • Emotional Support Animal (ESA) letters.

    • Psychological evaluation for bariatric surgery clearance.

    • Psychological evaluation for gender reassignment surgery clearance.

  4. You may be charged an hourly fee, outside of insurance billing of appointments, for completion of:

    • Disability paperwork.

    • Legal forms/reports.

    • Collaboration with other providers.

Administrative Policies

We are committed to providing you with the best possible care. If you are covered by medical insurance, we are happy to help you receive you maximum allowable benefits. In order to achieve these goals, we need your assistance and your understanding of our policies.

  1. We must emphasize that, as medical providers, our relationship is with you. Your insurance is a contract between you, your employer (if applicable), and the insurance company unless we are contractually participating with your insurer. As a service to our patients, this office will bill all primary insurance policies, provided we are given the appropriate insurance information and releases. While the filing of insurance claims is a courtesy that we extend to our patients, all charges are your responsibility from the date services are rendered. If we are not participating providers with your insurance plan, we will accept assignment of benefits in most cases, providing the applicable co-payments and deductibles are paid at each visit. Any balance not payable by the insurance company will be the responsibility of the patient. It is the responsibility of the patient to advise the office of any changes within your insurance plan or any change of insurance carriers prior to your appointment.

  2. The providers on our staff are participating with most major insurance carriers. As participating providers, assignment of benefits will be released to this office. All co-payments and deductibles due must be paid at the time of each visit in accordance with your insurance contract. As participating providers, we will collect up to the allowable amount as stipulated by our contract with the insurance company.

  3. Payment is due at the time service is rendered. Failure to pay your fee responsibility after (2) visits will result in the postponement of visits until your account is current. Account balances are considered delinquent after 60 days and may be subject to collection with a $25 fee. Interest charges of 1.5% per month may also be assessed. A $15 fee will be charged for each check returned for insufficient funds. If difficulties arise paying personal balances, you should contact the Accounts Manager to make financial arrangement. Some fee adjustments may be made depending on income and circumstances. Our regular fees are as follows: Individual, Couples, and Family Therapy: $165. Medication Management: $135-$155 (complexity-based).

  4. During or after office hours, when the need for extended telephone sessions with providers arise during urgent or crisis situations, the fees for this service may be charged directly to the patient if not covered by insurance plans.

  5. You will be charged a fee of $25 for your reserved appointment if the office is not notified of your cancellation 24 hours in advance. Your insurer does not cover this charge and you will be responsible for this payment prior to your next visit. This charge is automatic; however, you may discuss it with the Office Manager in accordance with each circumstance.

  6. Our Physicians, APRNs, and PAs provide psychiatric medication management. You will be advised of the possible risks and/or side effects of each prescribed medication. If you are seen by a mid-level provider (APRN or PA), please be advised that there is a collaborative relationship between the Physician and mid-level provider, which does not constitute supervision or direction by the Physician.

  7. In the event of an emergency or crisis after office hours, you are directed to report to your nearest emergency department or call 911. Additionally, Portal Messages are not checked regularly. THE PATIENT PORTAL IS NOT TO BE USED FOR URGENT OR EMERGENT MATTERS. Please call the office main line for immediate assistance with refills, rescheduling, or to discuss any other matters.
